Application Sent!

If you are a club-sponsored applicant, YOU MUST provide your club contact with a copy of your Application after you print it out. Be sure to include all the additional required Documents with your application. The chapter will forward the documents on to the AMBUCS Resource Center.

Semi-finalists will be notified by mail between January 31 and April 30.

If you are chosen as a semi-finalist, you will be required to furnish the following required documents:

IRS Form 1040 (both sides) from 1999 tax year. Include
parent or guardian 1040 if you were reported as a dependent.
Personal statement. A typed statement of not more than one page double spaced which describes the development of your interest in your chosen field of therapy; your plan of study; career plans after graduation; and why financial assistance is needed.

After you receive notice that you are named a semi-finalist, Send the required documents to:
